If space is limited, preference will be given to patients who have been an assigned patient of one of our clinicians in the past 2 years.
Others will be added to our wait list as we grow our team.
We offer primary care services (preventive care, chronic condition management, acute concerns, and some office procedures) for patients of all ages.
We are working hard to be able to accept most insurance plans and will bill the insurance company on your behalf. Visits and services are billed to insurance and typical copays and deductibles apply. The monthly membership fee enables us keep our practice independent and small enough to offer the experience you are looking for.
We offer both a Core Membership and a Premium membership option. With either option, we are committed to you being able to schedule timely visits with your care team.

Membership Subsidies
We offer reduced memberships for qualifying patients.
We’re a mission-driven clinic in startup mode. While our target level of subsidies aren’t yet available, we believe in healthcare access for everyone, and are working to raise funds so that nobody is turned away long-term.

At JoiViva, we believe that personalized primary care should be available to everybody, that primary care can be financially sustainable, and that clinicians and staff can thrive. We're here to prove that.
We recognize the nonprofit service structure we have built to enable membership subsidies is untried and likely to evolve as we learn. We welcome those learnings and welcome you to join us on the journey.
